For you to begin work and see patients, you must have obtained the following:

  1. License to practice medicine in the State of Washington
  2. Medical staff appointment(s)
  3. Faculty appointment
  4. Membership in the physician's practice plan

Drug Enforcement Administration (DEA) registration

Maintaining a current individual DEA certificate is a requirement for all medical staff - including residents and fellows (GME DEA Registration Policy) - who prescribe, order, administer, or handle controlled substances. Providers can apply for and renew their license online at: http://www.deadiversion.usdoj.gov

All providers at HMC and UWMC are fee-exempt, as UW Medicine is a Washington state agency.

Required training

In addition, you need to have completed any required training. Please work with your division to complete all requirements.

Sites of practice

NOTE: All sites of practice, including any at which you engage in volunteer activities, must be approved by the dean through the school and practice plan site of practice approval process. 

The medical malpractice liability coverage provided for you through the University and practice plan will only apply to these approved sites of practice where you practice as a University of Washington faculty clinician.
